Members may be required to present their Vitality Membership card at time of check-in.
Members are responsible to notify the clinic in writing of their change of address or change of credit card information for electronic funds transfer (EFT) which are scheduled monthly. Simply go to the front desk and complete and return a Payment Authorization form.
You have the entire term of your membership to use your pre-paid chiropractic visits. Any unused visits will expire should you elect not to renew your membership or once you enter a new auto-renewing year of your membership.
